Using "Staff Only Edit" Options in Forms to create Incident/Injury reports and Permission Slips

You can use Transparent Classroom to digitally assign and collect forms like incident reports and field trip permission slips using the "Only Staff Edit" option on Normal Forms.  

The Staff Only Edit option allows staff to add information to a Normal Form before assigning to the family. This allows you to send and collect forms with custom or unique information digitally.

Finding and Enabling Staff Only Editing

When you're creating a Normal Form, you can make any of the following options Staff Only Edit: 

• Yes/No Question
• Dropdown
• Checkboxes
• Multiple Choice
• Paragraph
• Date
• Signature
• Child Birth Date
• Child Gender
• Allergies
• Dominant Language
• Ethnicity

To make any of these options Staff Only Edit, check the box that says "Only Staff Can Edit" or click the button and select "Only Staff Can Edit" and then check the box next to that option before clicking [Done].

Tip: You can also make Staff Only Edit options "required". This means that staff must fill out the field before they send the form to families. 

Using a Form with Staff Only Edit Options

When you assign a form to families with Staff Only Edit options, you will be prompted to enter information before you add the form. Staff view looks like this:
